The compulsory medical insurance policy must be replaced immediately after receiving a civil passport. What is a compulsory medical insurance policy and why is it needed?
A compulsory health insurance policy is a document that guarantees the receipt of free medical care in the health insurance system throughout the Russian Federation. You can receive free medical care only in those institutions that have signed a cooperation contract with an insurance company. The following can receive a compulsory medical insurance policy:
- citizens of the Russian Federation;
- foreign citizens permanently residing in the Russian Federation;
- refugees;
- stateless persons.
Citizens of the Russian Federation receive a uniform insurance policy with no expiration date. All other categories of citizens are issued a document indicating the time of its validity.
Military personnel do not take out a policy because they have their own medical care system, which is paid from the state budget, and not through compulsory medical insurance.
Having a compulsory health insurance policy, you can count on the following package of services:
- free treatment throughout the Russian Federation (if the case falls under the compulsory medical insurance system);
- free medical examination for early detection of chronic and oncological diseases;
- preventive examinations;
- free medications (which are included in the list of vital medicines).

In 2020, a pilot version of the program for replacing insurance through Gosuslugi was launched. Residents of St. Petersburg were able to use it. The service immediately became popular, which is explained by the simplicity of registration:

List of required documents
To replace a compulsory medical insurance policy, you need to collect a certain package of documents:
- Statement;
- Passport with a changed surname or birth certificate for minors;
- SNILS;
- Marriage/divorce certificate or other document confirming the change of surname;
- Old compulsory medical insurance policy (optional);
- A document confirming registration in the Russian Federation.
In the application for replacement of the compulsory medical insurance policy, you must indicate:
- name of the branch of the insurance organization;
- Full name of the applicant;
- a request to replace a medical policy due to a change in surname;
- information about the insured person, his status: employed, unemployed, permanently or temporarily residing on Russian territory, etc.;
- applicant's gender;
- Date of Birth;
- Place of Birth;
- passport: series and number, date of issue, who issued it;
- address of registration and permanent residence;
- information about the representative of the insured person (if necessary);
- the presence of family ties between the representative and the insured person;
- contact telephone number and signature of the applicant.
In the received application, the insurance company specialist will write a note of acceptance indicating the date, and will also indicate the number of the temporary certificate that was issued for the period until a permanent one is received.
All documents are submitted in original with photocopies attached. But if an application is submitted to the insurance company branch by an authorized representative of a citizen, then he must have a power of attorney to represent interests.
Replacing a compulsory medical insurance policy when changing your last name at the MFC

The need for exchange and timing
The compulsory medical insurance policy is a guarantee of purchasing medicines and receiving assistance free of charge, indicating personal data, issued to each insured citizen in the compulsory insurance system.

In what cases does it need to be changed?
In October 2020, a message from the Federal Compulsory Medical Insurance Fund with a proposal to rush with a replacement made Russians worry, but later representatives of the fund clarified that the right to medical care under old-style policies is retained for an unlimited period of time. Medical care is guaranteed to every citizen of the Russian Federation and can only be lost if Russian citizenship is terminated. Replacing the policy should be mandatory for those who want to change medical insurance organizations or often move to different regions of Russia. Since changing an insurance company is possible once a year, the issue of switching to a new insurance company should be taken responsibly, choosing organizations that have successful experience in protecting the rights of their insured.
In 2020, there are no changes regarding the need to replace old-style policies with new ones, which means that the rules of service in clinics under compulsory medical insurance policies remain the same.
If the policy contains current information about the person (full name, date of birth, gender), there is no need to change old policies issued before 2011.
It’s another matter if the policyholder has changed his last name and presents the old policy when contacting a medical institution. In such a situation, a citizen with a new surname is obliged to change his passport within 30 days and then all personal papers used constantly (licence, SNILS, medical policy). According to current rules, replacement of the policy is required in the following circumstances:
- Changes in personal data that led to the need to replace a passport,
- There are errors in the compulsory medical insurance policy,
- Damage or loss of the policy.
Even if the policy turns out to be invalid, this fact will not become a reason for refusing to provide emergency assistance to the patient.
Reasons for refusal and solutions
As such, there are no reasons for refusing to provide the service; a multifunctional center specialist does not have the right to refuse to accept an application and documents according to the list.

If emergency medical care is needed due to a sudden serious illness, poisoning, injury, etc., threatening a person’s life, a medical organization does not have the right to refuse to provide assistance in accordance with paragraph 1 of part 4 of Article 32 of Federal Law No. 323, even in the absence of a compulsory medical insurance policy.

Validity
There is no specific validity period for the policy, as it was issued at different times in different regions. For example, electronic cards appeared in the country only in 2011. Paper documents are issued with a certain validity period, usually ranging from one to five years.
As for the new type of policy, as a rule, it is of an indefinite nature. Currently, it is issued to those who receive it for the first time. For example, this includes newborns and uninsured individuals.
Many people do not know how long their certificate is valid. In fact, it’s quite easy to find out the deadline - it is indicated on the front side of the document. It is worth noting that the unlimited option is the most profitable, as it greatly simplifies the life of its owner.
There are also several specific conditions under which you will need to replace the policy before its expiration date:
- as a rule, girls change their surname after marriage; accordingly, it is necessary to change all documents, including medical ones;
- in some cases, the policy changes when the passport is replaced, for example, upon reaching a certain age, its loss or damage.
Having an expired policy is not a reason for doctors to refuse medical care, however, it may complicate the preparation of certain documents.
Temporary certificate
A temporary document is no different from a permanent document in its capabilities. Throughout Russia, anyone with a temporary document can use free medical services.
The document allows:
- attach to a clinic;
- receive dental services;
- sign up for an appointment with the doctor;
- enroll your child in school or kindergarten.
The main discrepancy is the validity period of the temporary certificate, which is 30 days.
There is no barcode on the temporary certificate, so you won’t be able to make an appointment with a doctor online.
Obtaining a policy
In order to obtain a compulsory health insurance policy, it is not enough to write an application; you also need to have the following documents with you.
The list of them is quite diverse and has its own characteristics in connection with the person who wants to receive it, so if you are a citizen of Russia and have already reached the age of 18, then you need to bring:
- Passport or any document that can prove your identity. If you are just processing a passport of the Russian Federation or are restoring it after a loss, instead of a passport you can present the insurance company with a temporary identity card. So, this document will be issued to you at the time of production of your passport.
- Insurance number of an individual personal account (SNILS) - this document is not mandatory, but its presence will greatly facilitate the registration process.

Replacement procedure
The new form of medical policy differs significantly from the old document. A modern policy has 2 options: plastic and paper. As a rule, a plastic document is issued in addition to a paper one, and the information contained in both types of policies is identical. A plastic card is more convenient for everyday use; it does not wrinkle or wear out.
The form of the compulsory medical insurance certificate has changed several times in recent years. At the moment, it is regulated by the Order of the Federal Compulsory Medical Insurance Fund of the Russian Federation dated August 15, 2000 “On the introduction of a unified system for generating a compulsory medical insurance policy number.” This order unifies the form of the insurance document.
The changes made to the compulsory medical insurance policy are designed to improve the unified database of compulsory health insurance and simplify the procedure for making an appointment with a doctor. And the plastic form makes the document more practical and convenient to use. The location of the policy details when considering the plastic option is as follows:
- 16 digits on the front side are the policy number itself;
- The 11 digits on the back are the details of the document form.
If a citizen does not have the opportunity to look at the document at the moment, but the data is needed, then you can find out the compulsory medical insurance policy number by last name and passport data. To do this, you only need Internet access from a smartphone or other device.

The details of the compulsory medical insurance certificate of the old form are located on the front side of the form (it is the only one) - these include 16 digits written in the line “Series/No.” The first 6 positions indicate the series of the document, and the next 10 - its number. To do this, you need to enter the numbers in the appropriate service columns on the website of the regional TFOMS. If the document is an old one, then you will also need the date of birth of the insured person.
This card is more convenient to store and keep with you; it is accepted in several countries.
If the insured person does not plan to change the insurance company, then the replacement procedure is extremely simple. To do this, you should contact the insurance organization that issued the old-style policy, providing the policy itself, an identity card and a pension certificate.
According to the law, document changes are free of charge. If the need to replace a document coincides with a change in any data, you must take with you documents confirming this fact (for example, a certificate of change of name/surname).
The insurance company will need to fill out an application and submit it along with the old policy, receiving instead a temporary certificate, the validity of which varies from 1 to 1.5 months. When the new card is ready, you will need to visit the company again and get a permanent compulsory medical insurance policy.
The new type of insurance policy is indefinite, but in a number of cases (change of full name, place of permanent residence, correction of incorrectly specified data, etc., as well as damage or loss) it will need to be replaced.
To do this, you must contact your previous or new insurance company and fill out a form indicating the reason for issuing a new policy (certificates or other legally binding documents confirming this reason must be attached to the application).
If the insurance policy has been lost, the company's account number is attached to the application, but if the document is in hand, it is submitted along with the application form. At the end of the verification period and after the policy has been issued, the client must personally come to the insurance company and, after making sure that the specified data is correct, receive a ready-made card.
This type of insurance document has a single form. Previously, the policy was a paper form, but since 2014 it began to be issued in the form of a plastic card.
This document must contain the last name, first name and patronymic of the insured person. This means that when changing personal data, including last name, a person must replace the compulsory medical insurance policy, since he will already have a different last name in his passport.

Replacing a policy through government services

The insured person is obligated to change the policy if personal data changes no later than 1 month from the day these changes occurred.
For example, a person received a new passport on February 2, 2020. He must contact the insurance company to replace the compulsory medical insurance policy no later than March 2, 2020.
Renewal of the policy at the MFC
In order to quickly and easily reissue a compulsory medical insurance policy, you can skip the queue at the insurance company, get a temporary policy and come back to the insurance company to receive a permanent document.
